What were you trying to do?

What were you trying to do while the bug happend
ex: i was trying to queue for a sword match while another player was already in that queue

What happened instead?

What happened instead of the thing you were expecting
For example:
Instead of getting in to a match i instead got kicked out of the server with the reason

You should say everything you did.

while the last parts are usually the reason for a bug to happen there are times that instead of the thing you currently did the things you did in the past caused the bug.
for example we had a bug that when you recived rank after you used command /playtime you would have beend kicked out of the server. while the user reported being kicked after using /playtime the user DIDN'T report that he bought rank when he joined the server.

details

in a lot of cases even the amount of chat you send while trying to do the thing could help
it may be an unimportant case BUT saying the details like how many people where in the party could always help the developers to fix the bug faster and without retesting again

Retrying the bug

if you could or could not do the bug again.
if you did the bug again please give the most important parts of the bug need for the bug to happen

An example of good bug report:
I joined the server and played couple of matches in a party
after changing the party setting from private to public I coudn't leave the party anymore.
I wasn't the party leader.
there were 5 people in my party.
and i was on the server for 2 and half hours.
I couldn't do the bug again.
